Withdrawal

Please complete the “Intent to Withdraw” form if your next school year plan has changed due to one of the following:

  • Geographic Exception approval

  • Acceptance to private school

  • Moving out of Ewa Makai Middle School area

This form must be completed prior to generating your child’s release records. Many schools will not complete your child’s registration without these items.

 To assure proper transfer of your child’s records, please mail, fax to 808-685-2052, email to g-296-em-registration@k12.hi.us, or drop off in person the completed “Intent to Withdraw Form ” and current government issued identification (for verification).

PLEASE ALLOW AT LEAST TWO (2) WEEKS TO PROCESS YOUR CHILD’S RELEASE PACKET.
