Return Federal Impact Forms to EMMS

If you have not yet returned your Federal Impact Aid Survey form to EMMS, please do so as soon as possible. This form was previously mailed to your home. See the image of the form below as a reminder of what it looks like. 

If you need another form you may pick one up in the office on school days 7:30 am - 3:30 pm. You may also contact the EMMS office at 808-687-9500 to request another form be sent to your home or with questions. The completion of these forms equates to federal funding for EMMS and the goal is to receive 100% of the forms back regardless of the data.
